Fees
Diagnostic Records Fee
This is a way of measuring and diagnosing your child's problem. It is necessary for medical and legal reasons, as well as for treatment planning purposes. Current, relevant material is necessary for this procedure.
Active Treatment Fee
This includes the placing of appliances, as well as maintenance and treatment of the appliances. It also includes any normal, minor emergencies and any necessary changes in appliances in order to facilitate the most efficient treatment. It extends over the entire period of time of treatment, unless lack of patient cooperation necessitates increased treatment time. An additional monthly fee may be charged if treatment time exceeds reasonable limits. You will be consulted by us in this matter before any charges are made.
Retention Phase
Upon completion of the case, appliances are removed, records are made of the finished case, and retaining appliances are placed. The patient is normally seen periodically over the next 2 years and the retaining appliances are adjusted at these visits. There is no charge for this service. There is a fee to cover any replacement retainers due to loss or breakage.
Breakage/Loss Fee
Lack of patient cooperation can result in breakage of appliances and extended treatment time resulting in higher costs. A fee will be charged for excessive breakage.
Treatment
Upon transfer to another orthodontist, assessment will be made as to the percentage of completion of the case. An adjustment by refund or payment of adjusted balance is expected at the time of transfer.
Contract
The contract arrangements to pay the balance of the orthodontic fee are merely a means of budgeting (without interest) the fee over a portion of the expected time of treatment. Treatment is completed as efficiently as possible regardless of the contract arrangements.
